This page is intended for care providers who wish to keep track of their patients' or loved one's progress. Our goal is to enhance collaboration with all care providers involved in a patient's recovery and well-being.
Primary physician and treating specialists: After logging in, you will see a list of patients who have identified you as their primary physician or a treating specialist on our patient registration form. Select a patient to view our notes for them. This includes the patient's initial evaluation and a progress report for each visit. In addition, you can provide feedback or ask questions directly below each visit's notes.
In addition to viewing progress reports, patients and caretakers can:
Patients: Gain access to your account by registering above. You may also allow access to family members or guardians, or restrict access to care providers at any time. If you wish to do so, please Contact Us.
Caretakers: To gain access to a patient's notes, please Contact Us.
